I believe that volume is the primary dynamic of youthful facial appearance. Older cosmetic procedures emphasized tightening facial skin which can flatten elastic soft tissue shape. Aesthetic surgeons should be sculptors of facial form. Fillers have added in-office convenience with limited recovery but reshaping should not be limited to the filling of shadows. Adjacent to the shadows of stretched ligaments are displaced bulges. This is clearly seen in the midfacial relationship of your nasolabial crease and fold. Our goal of surgery is to make patients look as they did at an earlier time of their lives. This point is worthy of reiteration. We should create the lightness and softness of youth. Cheek implants greatly assist in obtaining the natural volume of expression of happiness and youth. This is referred to artistically as the “OGEE curve”. My work with cheek implants has been selected for presentation at the Annual Meeting of the American Society of Plastic Surgeons. I have defined four groups of patients with specific indication for cheek augmentation either as a sole intervention or as an essential part of rejuvenation surgery. Some people have midface volume deficiency due to age-induced soft-tissue descent and deficiency. Others had prior facial rejuvenative surgery that emphasized stretch and removal of soft tissue volume by liposuction resulting in a gaunt unhealthy appearance.. The third group is defined by significant asymmetry which can be developmental, traumatic or due to disease or paralysis. The fourth group is defined by any male who requests facial cosmetic surgery. Men, in contrast to women with lovely heart- shaped faces, should have a strong square shaped masculine appearance after surgery. Many men request treatment of the turkey gobbler neck which limits the accentuation of the strong mandible. Once corrected, the neck shape and jaw line can make a flat cheek look even more hollow.This lack of appreciation of facial harmony is evident in scary photos of many of the most popular Hollywood stars! With presurgical planning based on skeletal and soft tissue proportion, a natural. more youthful result is obtained. Cheek implants should be considered in each of these patients.
Cheek implants are used to improve the contour of the face, particularly in men and women with long faces cheek implants can help to fill out the face. Additionally, as we age our skin looses its elasticity, as well as valuable fat, and muscles can begin to sag, all of which works together to create that sunken in appearance. Cheek implants will often be placed in conjunction with facial rejuvenation procedures such as a facelift to create a more smooth and youthful appearance. Cheek implants can increase facial harmony and create a more aesthetic balance among your other facial features.
Cheek implant surgery can be performed under local anesthesia with a sedative or under general anesthesia, depending on the patient and Plastic Surgeon’s preference. Implants will vary in size and shape to meet each patients individual goals. The procedure will take approximately 30 to 45 minutes when performed on its own, though it may be combined with other facial cosmetic procedures such as a brow lift, eyelid lift, or facelift.
As with any surgical procedure, there are risks. Your Plastic Surgeon will go over these risks with you, but should you have any questions it is important that you ask them. There will be some dietary and activity restrictions following your procedure that you will need to follow while you heal to ensure a safe recovery. You may have limited mobility in your mouth and lips immediately following the surgery and any stitches placed to close incisions in your mouth will dissolve on their own within 10 days.
Chin implants are used to create a more aesthetic balance to the facial features. The chin and nose directly effect each other and one can cause the other too look disproportionate. A chin implant also helps to define the jaw line to create a more clear distinction between the chin and the neck. Often a chin implant may be performed in conjunction with other facial procedures such as rhinoplasty (nose surgery) or liposuction of the neck. Chin implants can increase facial harmony and boost self-confidence in people with a weak chin.
Chin implant surgery can be performed under local anesthesia with a sedative or under general anesthesia, depending on the patient and Plastic Surgeon’s preference. The incision may be made under the chin or inside of the mouth and the implant will be placed directly on the original chin bone. Any scarring will be hidden. The procedure itself takes approximately 30 minutes to an hour. The chin implant size and shape will vary based on each patients individual needs. Sutures placed on the outside, on the skin, will be removed in 5 to 7 days, while intra-oral incision (inside the mouth) sutures will dissolve on their own.
As with any surgical procedure, there are risks. Your Plastic Surgeon will go over these risks with you, but should you have any questions it is important that you ask them. There may be some dietary and activity restrictions following your procedure that you will need to follow while you heal to ensure a safe recovery. It’s normal to have some bruising around the chin and neck, have difficulty smiling and talking and to experience some slight swelling and discomfort around your face while you heal. Should you experience difficulty breathing or chest pains, you should seek medical attention immediately.
Your results will continue to evolve for several weeks and even months as you continue to heal. In most cases, the improvement is subtle but substantial, meaning others may not be able to pinpoint the change, but instead will be able to note an improved confidence and facial aesthetic.
